Eligibility to Apply for an AsIAm Autism I.D. Card:
Applications for the AsIAm Autism ID Card are open to all residents of Ireland (including Northern Ireland).
We will accept any clinical confirmation of diagnosis, regardless of the applicant’s nationality, provided the evidence provided is official and accredited. Any fraudulent information will qualify for immediate disqualification from the Scheme and possible legal action.
If your application is successful, we will issue a card. By accepting the card, you agree to be bound by the Terms and Conditions as amended from time to time. This is in addition to your acceptance of the Terms and Conditions as set out upon application.
We will not be responsible for late, lost or misdirected post (either received by us or sent by us to you).
The purposes for which Data may be processed by us include:
To validate your personal details.
To validate the applicant’s personal details.
To process and print the autism I.D. card.
To retain information for ‘opt-in’ sections of the application form which include, but are not limited to, subscribing to the AsIAm Newsletter and related bulletins and the participation in a survey on the autism I.D. cards.
We may use this information to contact the applicant in the event of further information being required.
We are using third party websites for the application process including but not limited to Typeform and Stripe. Information will be carried by these websites for the sole purpose of processing applications.
For verification purposes, a spreadsheet will be retained of all cards issued. This is to prevent the duplication of cards and the return of lost cards. This information will never be shared with a third party.
AsIAm may periodically release statistics around the number of cards issued and the demographics involved but will never release any personal information. These statistics may be used to inform AsIAm’s programme of work, advocacy work or publicity materials.
We will notify you 30 days before the expiry of your card with a reminder to apply to renew the card. If you fail to respond within 90 days of expiry of your card, all data provided by you will be deleted.
Right of Deletion:
In the event that you wish for your details to be deleted from our system, you should return your card with a note requesting the deletion of your data.
In the event that you have lost your card and wish for your data to be deleted from our data records, you should present a written letter requesting the deletion.
Applicant’s obligations:
The applicant must consent on the application form that they acknowledge that payment to the company covers the cost of the card application processing and validation service. The applicant is not paying for the Card which remains the legal property of the company.
Falsifying original ID, for example using software such as PhotoShop®, to doctor a government-issued document such as an original or certified copy of a birth certificate, passport or any other government-issued ID is a serious criminal offence and the company will always refer evidence of such to the Gardaí.
We do not accept responsibility for the loss, theft and or destruction of any issued cards. All lost or stolen cards must be reported to AsIAm as soon as possible.
Any cards found by third parties should be returned to AsIAm headquarters at the registered address at Rock House, Main Street, Blackrock, Co. Dublin, A94 V9P1.
We expect that all cards will be used in good faith and in line with the spirit and ethos of the scheme.
Use of card:
Although we may issue you with a card, it remains at all times our legal property. It may only be used by the lawful holder of the Card who is the applicant.
The card may not be lent, sold or given to any other person under any circumstances.
The access needs of no two autistic people are the same. The strength of this scheme is based on its ability to provide access and priority services for cardholders. As a result, cardholders should only avail of supports and priority services that may be made available where their own access needs require it.
Only one card may be issued per individual. This is to hold the integrity of the card and prevent misuse.
The AsIAm Autism I.D. Card is a means of providing staff with an explanation of the cardholder’s diagnosis and the nature of their challenges. It does not automatically provide discounts and or preferential treatment over other customers.
All supports rendered are entirely at the individual discretion of the business or service provider.
AsIAm does not take responsibility for the standard of access supports services provided by a third party.
Where a third party does put in place additional benefits or privileges for Autism I.D. Card holders, we reserve the right to receive complaints about cardholders and to empower the third party to retain the card and return it to AsIAm to consider a complaint. Should this happen we may contact you and the third party to discuss the incident and may cancel the card if we conclude that the card is being misused or fraud has been committed.
The I.D. Card identification numbers may be shared with third parties for the purposes of allowing additional benefits and privileges to be provided for cardholders. However, third parties will not have access to any identifying information on AsIAm’s database.
This card is issued by and is the property of AsIAm, Ireland’s National Autism Charity – on receipt of a diagnosis. Only the authorised card holder and a companion may use this card in accordance with the current conditions of use issued by AsIAm.
Letter of Diagnosis:
The purpose of the applicant’s diagnosis letter is to prevent abuse and improve the integrity of the card.
A copy of an autism diagnosis or a letter confirming the diagnosis will be accepted from the following professionals: GP; Paediatrician; Psychiatrist; Clinical or Educational Psychologist; Speech and Language Therapist; and or Occupational Therapist.
We respect that the fact that there are many autistic individuals who have not yet received a diagnosis. We nonetheless insist, that to protect the integrity of both the application process and administration system, all prospective applicants for the ID Card must provide evidence of an official diagnosis from an accredited healthcare professional.
